What We Did
Following an initial home visit and discussion with the family, she joined as a full-time residential resident. Her care plan was developed over the first two weeks as the team learned her preferences. Written updates were sent fortnightly to all family members in two cities.
Outcome
After 14 months as a resident, the family reports that she is noticeably more engaged and less isolated than she was living alone. She attends communal activities four days a week. The family's peace of mind, they say, has changed significantly.

Recovery After Hip Replacement Surgery
The Situation

What We Did
He joined the rehabilitation programme two days after hospital discharge. Daily physiotherapy began with weight-bearing exercises under the physiotherapist's guidance. Progress notes were shared with his surgeon. The stay was extended from four to six weeks at the team's recommendation.
Outcome
